Neutropenic Day 9

Avery's pain was better managed last night, but she was still in a good deal of pain. The pain has her senses heightened, so every vital check, line hook up, and med push that occurred during the night woke her up and made her very angry.

Her hemoglobin was 7 this morning, so she required a blood transfusion. Her platelet count was still good. Her ANC had risen to 10 with some monocytes present, so hopefully we are moving in the right direction. Her sodium was 136, so they added back in a 3rd dose of oral sodium a day(3g at 3 times a day). She continues to remain fever free. Based on her pain and how quickly the medication was wearing off. they increased her dose again to try and keep her more comfortable.

She began the day by vomitting, but luckily didn't have anymore nausea throughout the day. Her pain still has her extremely angry and uncomfortable. She doesn't want to be touched or talked to which is impossible in a hospital setting. Her hair is falling out quickly and getting in her face and mouth, which just adds to her anger. I am hoping we will shave it this weekend sometime, but we need her in less pain and a more cooperative state before we can do anything.

They ordered a CT of her pelvis and right upper leg to look for the source of the pain. They were able to wheel her down in her bed to avoid the pain of being carried, but we still had to move her onto the CT table. Laying flat on her back with her legs straight(as needed) put her in unbearable pain. They taped and strapped her legs down. Shon held her lower legs still while I held her arms above her head for the scans. She fought the entire time and gave us all a good workout. Although the pictures were somewhat blurry from her moving, they were able to identify a 2x2 cm abscess in her pelvic area that is causing all of her pain. They switched her antibiotic to Clindamycin to better treat the infection. She will have surgery Saturday morning to drain the abscess. Although she will have some soreness from the incision, they expect almost immediate pain relief for her.

Despite all of the trauma and pain of the day, she once again blessed us with her sweet smile for a moment. She was finally able to rest some this evening. We are hoping for an uneventful night and looking forward to getting her some relief tomorrow.

- Successful and safe surgery(she's at a much higher risk of additional infection due to her low ANC)

- Pain management

- ANC to begin rising

- Wisdom and guidance for the next steps of treatment

Praying for all the things you are asking for. Sweet little beautiful bravery is such a fighter. No matter how she feels she always has a big beautiful SMILE 😃. Love this tough little girl and her family. Prayers for complete healing and a successful surgery to relieve her pain. 🙏🙏🙏
